GuardianHeart was incorporated in Arizona in 2026 after the founders watched a family member cycle through three assisted living facilities — each with paper charts, separate medication workflows, and different definitions of "compliant." The care was not bad. The people were not negligent. The system was broken.

Probabilistic AI tools promised to fix it. They didn't. GuardianHeart was built to answer the question "why?" before a regulator ever asks it.
